Hello Rick...  What value are the feedback caps?  Mine are .0022, and the 
output from each section is through a .1uf cap.  The first and last sections 
also 
have a resistor to ground at the output of the .1uf cap.  For some reason, 
the 2 middle output caps don't have the resistor.

If you can span the pic, that would be great.
