If I were you I'd be very reluctant to extend the top section.  It cost me a
mounting frame and antenna damage.  It peeled the two hinge plates on the flat
side like a banana. It all depends on the weight on the top section.
 
I had the same conflict of interest - a small fruit tree - that no longer is
present.

Is it OK (i.e., feasible, safe) to start tilting over a UST tilt-over 
crank-up with the tower still somewhat extended (e.g., an HDX-555 still 
10ft above its lowest position) -- and also to extend it by 10ft or so 
again before it gets back to vertical?

Without going into detail, the reason would be to clear a tree. The 
alternatives would be to trim the tree somewhat drastically or to try to 
synchronize turning the antenna and tilting the tower.
