I much prefer to deal with manual questions and issues off the reflector. 
But...

The quick answer is No, I don't see how to do this. 

The program that does the formatting has no concept of what is actually on 
the page. It sees boxes where the characters are, but has no idea of the 
glyphs contained in the boxes. Therefore, I don't see any way to implement 
a mechanism such as this.  

Also, adding pages with "peculiar" numbers such as 2-A would undoubtedly 
hose the index-maker. (Even if I knew how to do it, which I don't :-) )
