It's only grayed out if CW Keyer type is NOT PC Generates. 

With the external keyers (W5XD, PK-232, WinKey, MK2R+) all mode 
PTT is enabled all the time (and controlled by the external keyer 
configuration). 

With RTTY, PTT is also available on the FSK port (set in RTTYRITE).
