I've released CT to ADIF Version 6.0.  You can download it from here:

     http://software.ad1c.us/CT_to_ADIF/

There are both DOS and Windows versions in the .ZIP file.  The web page 
now contains a revision history at the bottom.

This is a major release, since so many features were added and so many 
bugs fixed.  If there's something that still needs work, please let me know.

Ver 6.0 - 27 October 2009

Features Added:

     Added window to show conversion errors

     ADIF header contains components of CT header information as well as
     QSO count

     Output of seconds in time is optional (default is ON)

     Output of frequency field (FREQ) is optional (default is ON)

     Added some state abbreviations for ARRL DX, DX-side

     DOS command line supports multiple options

     Added CONTEST_ID for ARRL VHF (Jan, Jun, Sep)

     Shows elapsed time to write ADIF file

     Converts NAQP name to UPPER/lower case (i.e. JIM -> Jim)

     Supports split-frequency operation by exporting FREQ_RX field

     Field Day class now exported to CLASS field (field day flag is gone)

     Exports CQ zone in DXped mode

Bugs Fixed:

     Abbreviation for South Dakota (SD) no longer treated as San Diego in
     CA QSO Party

     Converts 7-land states when logged with correct abbreviation during 
NEQP

     Fixed some stuff having to do with VE provinces

     If output file specified with .adi extension, output file no longer
     gets .adi.adi extension

     For some contests, if the state that was entered was not a 2-letter
     state abbreviation, the state (not abbrev) would be written to the
     STATE field

     Program could crash if prefix could not be determined (i.e. VP2V/N6LL)

     Empty .BIN file no longer crashes program

     Unknown DOS command-line option displays usage message, not "can't
     open input file"

     Frequency not "made up" if it's invalid (band is still output)

     Help button goes to correct URL
