Yes, I have seen that happen a few times.  No explanation any of the times
and after a power cycle, all was well and is still working (knock on wood) I
attributed it to fluctuations in ac power.  In the one location where I had
this happen a couple of times, I installed a quality UPS that regulates
voltage and have not had a problem with it since.....but it was a hunch
really, no empirical data to back that up.
