[RTTY] Log submission question for CQ WPX contest

How does one submit a log for the CQ WPX contest? I know how to attach a 
Cabillo file, but where do the Off-times go? The rules say:

"Off time periods must be a minimum of 60 minutes in length and must be 
clearly marked on the Summary Sheet."

But where does the summary sheet go?
