It is simply a question of mailing a copy of the license... but you're 
right, many hams haven't bothered.

Outside of the USA, there are a few individuals accredited by ARRL as 
QSL card checkers. If they were also accredited to verify license 
validity for LoTW within their own countries, it would bring a huge 
boost to the LoTW database.
