An other option I have used is a thumb ball.  The thumb tendon doesn't 
go through the carpal tunnel and you don't have the issues. I like it 
but it takes getting use to.  It DOES have the advantage on a cluttered 
desk that the "mouse" doesn't move...  As should any track ball.
