Here's an excellent reference from Nautel, a respected broadcast 
transmitter manufacturer. A copy of this paper is shipped with every 
transmitter, and it is considered to be a standard reference in the 
broadcast industry. This link is to the actual document, which is a 5.5 
MB PDF.
