A good way to think of coax is as always made up of 3 conductors:  The 
center conductor, the inside of the shield, (and those two are where the 
differential current flow happens,) and the outside of the shield, where 
common mode current could flow back to the shack.
The late W5GI used to start his antenna presentation by asking the 
question, "How many conductors does your Coax have?"
