An open shield connection or water in the cable can cause 
reactance to appear over a wide frequency range. 

Additional problems are dc or low frequency AC getting into the 
meter input, or the unit is out of calibration.

On more recent 259's you can check for external AC or DC signals 
by going to the "frequency counter mode" and looking at the SWR 
meter. If you have any deflection you have external signals. You 
can check calibration using non-inductive resistors.
