I think Riggers was at fault. They were well aware of the risks and
reacted to time pressures. Antenna Enginners should have considered the
needs of Riggers in the initial design, especially given their prior
experience. However Riggers signed off the job and accepted the
responsibility to construct the tower. Riggers should have consulted a
mechanical engineer prior to cobbling up a makeshift bracket.
