I use the holes in the top plate for guys which is why I suggested placing 
the top guy at the top and the tower is somewhat stronger by doing so. 
Personally, I drop the top guys when installing antennas with this 
configuration so working around them is not an issue.

Unless the tower was heavily loaded and therefore required it, I would
never put the guy at the very top. It is MUCH easier to work around the
top of the tower if the guy is 5 feet down.

...If you are only using the top of your tower, I would place the
guys at 30', 60' and 90'.
