I did actually see this once when I put in a DC grounding
coil on a vertical in a hurry and the VSWR would go haywire
at over 100 watts.  However, this was a really inadequate
coil.

The SWR meter on my FT-1000 has always been inaccurate at QRP
levels, the opposite problem.
