You know,

My self and a friend were battling just such a flash over problem in a 6M amp that turned out to be the radio 
putting out a pulse of RF sometimes when it was keyed.

Just out of curiosity, does this problem occur if you just run the tube from cutoff to operational bias without the 
exciter being active?

Just a thought....
