Also, along the lines suggested by George, K6GT, the usual zener diode 
mounting insulators are fairly fragile and can allow a sneak cathode to ground path.

This typically happens as a result of a plate to ground arc, forcing higher 
than normal voltage to appear on the b-/cathode circuit.
