I would very strongly suggest you use the actual base as your 
"template."  Either by buying a new one or, best of all, using the 
base from your tower.  You could do this by either waiting to pour 
the base until you move or by dropping the tower down early and 
hauling the base to the new house.

The tolerances are very close and I doubt either cardboard or plywood 
would allow you to even come close.  I can think of few things worse 
than your showing up with your tower only to find our old base won't 
fit the new bolts.
