[TowerTalk] Needed - US Towers MA-40 base template

I recently bought a US Towers MA-40 mast with the tilt-over and crank-up
base.  I need the wooden template for the anchor bolts or perhaps for
somebody to trace the template for the anchor bolts so I can prepare the
base.  Can somebody please help me?
