They're installed at your service entrance (main breaker).. 3 wires, 1 
to each hot, 1 to the neutral/ground (since at the service entrance, 
neutral and ground are bonded together)..

Formfactor wise.. there's two popular styles.. one is a blob that sits 
inside the box (which would be a tight fit in most residential 
installations that hadn't left room for it) and the other is a small box 
that has  1/2" conduit nipple sticking out that you install hanging off 
the side of the box.  If you slog through the Eaton/Cutler Hammer 
website and look at the "Advanced accessories" or something like that, 
there's a pretty good brochure.

I suspect there's other form factors, but those are the two I've seen.
