Thanks for all the feedback and confirming what I suspected with the weld and what I was seeing.   I could not have asked for a better controlled drop when the ear broke.

I'm going to pass on getting this base fixed.  Based on this weld...I'm not sure I can trust the other welds on the plate.  I have a few options right now for a new base...so I should be fine.   I'm emailing US Tower as well and see what they might be able to do with replacing the two legs of the raising fixture....and / or replacing the base.

A good welder should be able to fix that.  The bent pieces are a 
straight metal tab welded to a box (square tube) member.   I think 
they/it could be cut off, a new bracket made, and welded in place.  The one might just need straightening.  You could probably have a welder look at it and give a price quote.  They can come out to the site, or if you can take the pieces in, it would be considerably cheaper.
