I am pretty sure that Flex radio use just the board from LDG in their
Flex-5000 and Flex-3000, which are pretty small boards.

Look at the 2010 ARRL Handbook, there is a discussion on using this tuner
as an input circuit on HF amps. Apparently LDG offered a circuit board
without cabinet for use as an input circuit in HF amps. The amp it was
used
in was designed by Jerry K8RA ( I think that's his call) He discusses the
use of it.
