I can tell you, at least on my watch, no contract or captive manufacturer could 
make any changes without our engineering.  I have dismissed such services for 
doing such.  We monitored them by actually buying our own products and do a 
manufacturing engineering audit.  I can not speak for other companies.

PS  This problem can happen here.......look at General Motors

This is why we had great products and I got paid big bucks

One point not realized by many.   Yes the original design may have been done 
by the JA engineering team.  But there may have been a re-design done by the 
manufacturing team to facilitate cost effective manufacturing.  These two 
teams, design engineering and manufacturing engineering, may not have a 
working relationship.  I've seen this happen too often with the smallest 
issue such as changing the brand of an IC or reducing the number of screws, 
some critical to proper electrical contact with regard to grounds and 
shielding.
