Never having been DX in a contest I can only speak from the USA side.

If the guy doesn't give his call by the second QSO I work him and ask him.  I 
get several different responses.

The first is just him giving his call...sometimes it's a dupe, sometimes it's 
not.  Either way it's no problem for me.  The second is a pissed off, "I just 
gave my call".   Yeah, but was that ten QSOs ago.  He thinks I'm a lid, but 
I'm not going to wait around a minute or two for him to give his call.

I understand there are several reasons for not giving the call....to keep the 
pile smaller because some guys just get tired of waiting and wander 
off.....and to save a precious two seconds for every QSO.

My best hour stateside was 255 and I think I gave my call after every QSO.  
It can be done and it sure does help to correct incorrect packet spots and 
wasted time from guys working you as a dupe.
