In factories with very high levels of QA and inspection, things still get 
changed in production. Sometimes QA know and agree but don't check with 
engineering. Even if they do, there are times when the original guy isn't there 
and the people looking at the proposal don't have the history or sometimes the 
documentation as to why things were done originally. And then the troubles 
start......Sometimes, changes are well meaning. like in the 500kHz marine Tx 
where production figured that steel screws around the variometer for PA tuning 
would be cheaper than the specified brass. Steel screws would be cadmium plate 
and passivated, so they wouldn't rust, so that would be OK, wouldn't it?
The way the variometer burnt was impressive................fortunately, on test 
and not in the field.
