[SD-User] SDI - IOTA Reference Files.

Some users have tested SDI and found that it didn't let
you log certain islands.

This was due to errors or inconsistencies in SDIOTA.CTY
 - where the continent ID for the country does not agree
with the continent in the corresponding island reference(s).

I've updated SDIOTA.CTY as follows.
