Both, but you should always ask if you're serious about
getting a good score.  Some DX stations will not send
QTCs unless they have 10 or more "spare".  Others will
happily send you a single one.

Yes, the number of QTC "sessions" with a DX station is
irrelevant - but 10 QTCs is the max from any one station.
