Are the serials always out by 1 in the + or - direction?

Using win-test I have seen it where the QSO number is one or even 2
different to the serial number.

If the exchange was sent automatically then the serial is sent OK
If someone sent the exchange by hand then it is possible to be confused and
send the QSO# not the serial.
I have seen it happen on SSB.

I agree with Clive there should be real penalties not just the removal of
the QSO.
