That seems clear evidence that the used grip is compromised, don't reuse
it. Follow the instructions.
Another inspection on the Phillystran - is the Kevlar fiber recessed
inside the sheath? If so the sheath has slipped/migrated on the fibers.
That may have compromised the new grip pull strength.
