I know of at least one that applied to be put on the list, but
received no response to repeated applications.  What should be
done in these circumstances?

How can ARRL be responsive and co-operative with regard to something
it doesn't control, and probably has no wish to?  Cabrillo is "owned"
by Trey, but he is not always able to respond to applications.

Isn't that the issue?
